St. Michael School, Greenville, has opened kindergarten registration for the 2014-15 school year. If you would like a personal tour of our kindergarten, or would like to have your child visit the classroom setting, contact the Director of Advancement Joanie Micsky at 724-588-7198 to make arrangements. (This applies to all grades – Pre-K through eighth grade also.)

A packet can also be mailed to you by calling and requesting one. We require copies of the child’s birth certificate as well as a full, up-to-date, immunization record. Other forms required for registration can be downloaded at the schools website at under FORMS. There is a $200 new family discount for all newly enrolled families, as well as financial aid applications being accepted from now until March 31.


Parents of children in Greenville Area School District who will be entering kindergarten in the fall are asked to call  Hempfield Elementary School at 724-588-1018 between the hours of 9 a.m. and 3 p.m. Feb. 24-28 to pre-register their child for the 2014-15 school year. Parents will be told when they can pick up the kindergarten packet that needs to be completed before kindergarten registration. Formal kindergarten registration will be April 28-30.

To be eligible for kindergarten, a child must be 5 years old on or before Sept. 1. Parents are asked to bring proof of child’s age (birth certificate) and immunization records as well as proof of residency. No child will be allowed to register without both proof of age and a complete immunization record.

Additional information will be given during the pre-registration call-in.


Brookfield Elementary School, 614 Bedford Road Southeast, holds kindergarten registration for children who will be 5 years old by Aug. 1 as follows:

• March 11, 9:30 to 10:30 a.m.; 1:30 to 2:30 p.m. and 4 to 6 p.m.

• March 13, 9:30 to 10:30 a.m.; 1:30 to 2:30 p.m. and 4 to 6 p.m.

These items must be provided to register a child: Official birth certificate with raised seal; Social Security card; Ohio driver’s license or Ohio ID of parent or legal guardian; if divorced, proof of custody; and current immunization records.

Two proofs of residency are required. Examples would be a current utility bill and lease or mortgage agreement. Those without one or both must complete and have notarized a verification of residency form.

Hermitage School District holds kindergarten registration in Artman Elementary Building community room as follows:

• Feb. 26, 9 to 10:30 a.m. for last names A-E.

• Feb. 26, 12:30 to 2 p.m. for last names F-J.

• Feb. 27, 9 to 10:30 a.m. for last names K-O.

• Feb. 27, 12:30 to 2 p.m. for last names P-T.

• Feb. 28, 9 to 10:30 a.m. for last names U-Z.

Parents must take their child to registration, which takes about an hour and includes screenings for vision, speech, language and school readiness and math. Children must turn age 5 on or before Sept. 1 to be eligible for kindergarten in the fall.

Parents must take their child’s state-issued birth certificate; immunization record; proof of physical and dental exams done since September; proof of guardianship or custody if applicable; and proof of residence in the form of a deed, escrow papers, mortgage book or statement, lease agreement or rental contract and landlord-signed rent receipt or two of the following: gas, electric, water, phone or trash bills; verification of social services; or unemployment or employment check. A driver’s license is not an acceptable proof of residence.

Immunizations required are: four doses of tetanus; four doses of diphtheria (one dose on or after the fourth birthday, usually given as DTP, Dtap, DT or Td); three doses of polio; two doses of measles; two doses of mumps; one dose of rubella (usually given as MMR), three doses of hepatitis B (one dose on or after the age of 6 months); and two doses of varicella (chickenpox) vaccine or history of disease.

Kindergarten registration for Lakeview School District for 2014-15 will be held March 4-5 at Oakview Elementary School, 1387 School Road. Families living in Fairview, Jackson, Lake, Mill Creek, New Vernon, Sandy Lake and Worth townships and Jackson Center, New Lebanon, Sandy Lake and Stoneboro are in the Lakeview School District.

To enter kindergarten, children must be 5 years old before Sept. 1.

Since all necessary information will be provided by parents, children are not required to be present at kindergarten registration. Parents must take to registration this information regarding their child: A completed registration packet, a state-issued birth certificate, proof of residency and evidence of required immunizations (three doses of hepatitis B vaccine, three doses of polio, two doses of MMR, four doses of DPT/DT – fourth dose on or after fourth birthday, and two doses of varicella or history of chickenpox.

No child will be registered without both legal proof of age and complete immunization record.

Parents may pick up a registration packet between 8 a.m. and 3:30 p.m. weekdays in the Oakview office. Parents must also contact the elementary school main office at 724-376-7911 to request that a packet be sent home with a sibling who attends Oakview. When picking up or requesting a packet, parents will schedule an appointment for a registration time for either March 4 or 5. The appointment will take only about five minutes for parents with the paperwork completed before arriving.


Students eligible to enter kindergarten for 2014-15 may register on March 4 and 5 at Mercer Area Elementary School as follows:

• March 4, 8:30 to 11 a.m. for last names A-E.

• March 4, 12:30 to 2:15 p.m. for last names F-J.

• March 5, 8:30 to 11 a.m. for last names K-R.

• March 5, 12:30 to 2:15 p.m. for last names S-Z.

Screenings for vision, speech and hearing, and kindergarten readiness will also be completed during registration. The entire process takes about 30 to 40 minutes.

Parents are required to show a state-issued birth certificate and evidence of these state-required immunizations: Four doses of DT (one after the child’s fourth birthday); three doses of polio vaccine; two doses of measles, mumps and rubella (first dose after the child’s first birthday); three doses of hepatitis B vaccine; and two dozes of varicella (given after the child’s first birthday) or proof of disease.

Two proofs of residency are required, including driver’s license, lease agreement, utility bill, etc. If a student does not reside with both parents, custody agreement or proof of guardianship is required.

Registration packets can be picked up in the elementary school office weekdays between 7:30 a.m. and 3:30 p.m.


The Sharpsville Area School District is holding kindergarten registration night for eligible students on Feb. 27. To be eligible for the 2014-15 kindergarten class, a child must live in Sharpsville, Clark or South Pymatuning Township and be 5 years old before Sept. 1.

West Middlesex

Registration for children entering kindergarten in the West Middlesex Area School District for 2014-15 will be 4 to 7 p.m. Thursday and 8 to 11 a.m. Friday in the Luther Low Administration Offices.

Children entering kindergarten must be 5 years old by Sept. 1. Birth certificate, immunization records and two proofs of residency, such as rent receipt and utility bill, are required at registration. Immunizations include: four DTPs, one of which must be after age 4; three polio vaccines; two MMRs; three hepatitis B vaccines; and two varicella (chickenpox) vaccines or written statement with date of disease.

Parents must take their child to registration for screenings. Kindergarten registration packets may be picked up at the elementary offices between the hours of 8 a.m. and 3 p.m. Monday through Friday or download a registration packet from the district website at


NEW WILMINGTON – Wilmington Area School District will hold kindergarten screening for Pulaski Elementary and New Wilmington Elementary attendance areas on April 23-24 at New Wilmington Elementary School by appointment only.

Children who will be 5 years old before Sept. 1 can be registered for 2014-15. Parents or guardians should call 724-656-8866, ext. 3000, immediately to make an appointment.

A packet containing all the necessary registration forms and appointment time will be mailed approximately the last week of March to parents who contact the elementary school office.
