As per discussion, vapor devices not approved for use in a retail food facility environment. PIC voluntarily discarded both gallons in the three-compartment sink upon discovery. 2. Observed the only walk-in freezer light to be out / not functioning during the inspection. Address as needed. 3. Handmade wooden ticket holder observed to be placed and duct-taped to grill-line make-table , in the kitchen area. The wood is not smooth & non-absorbent. The duct-tape is visibly frayed and soiled; not designed and constructed to be durable and capable of retaining it's original characteristics or easily cleanable. Address as needed. 4. Exterior top of the low-temp dish machine observed to be soiled with food debris and dust. Showed PIC what needs cleaned.

The container was swollen with black mold visibly growing inside. PIC immediately voluntarily discarded the product using the outside dumpster. 2. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Inspection records show that employee had a ServSafe FEC that expired on 8/25/19. No documentation could be located or provided during the inspection to show the facility as having a certified food employee with a valid certificate.

GROVE CITY FOX'S PIZZA DEN

Violations/inspector instructions: 1. Table-mounted can opener in kitchen area is creating metal slivers and not cutting can surface effectively, as evident by the accumulated slivers around the blade assembly. Showed Owner. Address as needed. 2. Observed the kitchen hand-wash sink to have no cold water, only hot, available when attempting to wash hands. An inadequate temperature of water may lead to poor hand washing practices and is not practical when the water gets too hot to safely conduct hand washing. An employee opened the nearby panel for the water lines and discovered the cold water had been shut off via pipe ball valve. Cold water availability restored for proper water tempering. 3. Observed two (2) cardboard boxes containing dough balls stored directly on the floor in the walk-in cooler, rather than 6 inches off of the floor as required. Owner removed the items to elevated storage during the inspection. 4. Observed the three-compartment spray wand handle, in kitchen area, to be wrapped in electrical and duct-tape. Used as insulation to prevent employees from burning their hands when hot water is being sprayed, as per Owner statement. As per discussion, Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Address as needed. 5. Plumbing system not maintained in good repair - observed rapid leaking from the cold water supply line / spigot connection, behind the utility mop sink, when the cold water is in use. Showed Owner what needs addressed. Service / repair as needed.

VILLA PIZZA

Fraying and soiled duct tape adhered to the corner of the door in an attempt to prevent panel separation. The interior of the door is not in good repair or easily cleanable. Showed PIC. Service / repair as needed. 2. Plumbing system not maintained in good repair - observed hot water to be leaking from the three-compartment sink spigot when the handle is positioned to closed. Showed PIC. Service / repair as needed. 3. Three (3) vapor (Dichlorvos) insect control devices observed to be present at the sales register, warewashing area, and hung left of the gas range stove. As per discussion, vapor devices not approved for use in a retail food facility environment. This is a repeat violation from last year's routine inspection. An employee removed and discarded the devices during the inspection. 4. Observed two (2) red plastic buckets, used for sanitizer solution, to be cracked with well worn and scored interiors; not in good repair or condition. Address as needed.
